Treatment of post mastectomy lymphoedema of arm with 5,6-benzo-alpha pyrone : a pilot study

5,6-Benzo-Alpha Pyrone (Coumarin) can reduce the high protein oedema fluid by stimulating proteolysis there by reducing the lymphoedema in postmastectomy arm swelling. We have conducted a pilot study with Coumarin in Twenty cases of postmastectomy lymphoedema of arm. Patients received 400 mg, of the drug daily for six months. During the study the measurement of the arm was taken at fixed points and the volume of the limb measured. After six months of therapy the excess oedema has reduced by more than 50 percent in 21 percent of cases. A minimal reduction of less than 30 percent of excess oedema was observed in 15 percent of cases. Further the symptoms like stiffness, hardness, heaviness of the limb were reduced in almost all the cases giving good symptomatic relief. One out of the twenty cases had developed nausea and dyspepsia which was controlled with H2 Blockers and was relieved. Hence we conclude that Coumarin usage results in slow but safe reduction of lymphoedema in post mastectomy arm swelling and symptomatic relief.
