Granulocytic sarcoma is an extramedullary tumour composed of immature granulocytic precusor cells. It usually occurs in association with leukaemia or other myeloproliferative disorders but can occur without overt haematological malignancy. We describe a case of granulocytic sarcoma of the rectum in a 63-year-old lady who presented with difficulty in defaecation and micturation. There was no overt leukaemia associated with the presentation. The case has been presented in view of its rarity and discussed in light of available literature.
