Comparative evaluation of ondansetron vs metoclopromide/dexamethasone combination as antiemetic in cisplatin based cancer chemotherapy.

Platinum based chemotherapy induced nausea and vomiting are major side effects and often significant cause of noncompliance in cancer therapy. The present study was performed to assess the potential of a newer antiemetic agent ondansetron against the conventional combination of metoclopramide and dexamethasone. Twenty two patients receiving cisplatinum (75mg/m2)on D1 were evaluated. Twelve patients received metoclopramide(1.5mg/kg)plus dexamethasone(16mg)every twelve hourly, while ten patients received ondansetron(8mg)every 12hourly on first day. The mean time of onset of nausea was significantly delayed in ondansetron group(10.33+/-1.52)hrs vs 4.61+/-0.99hrs,P0.001)the mean number of emetic episodes was also less in ondansetron treated group (0.44+/-0.22vs2.08+/-0.49,P,0.01). Complete control of emesis was achieved in 70 in odansetron group compared to 25 in metoclopramide dexamethasone group. We conclude that ondansetron is superior to metoclopramide and dexamethasone combination in control of cisplatinum induced nausea and vomiting.

Lactate dehydrogenase and its isoenzymes in serum from patients with neoplastic disorders.

A study of lactate dehydrogenase (LD)and its isoenzymes was undertaken in the sera of patients with neoplastic disorders. In recent years, the relationship between neoplasia and LD has been studied with increasing intensity. The present study included 161 patients with neoplastic disorders, classified as Multiple myeloma (42),Gastro intestinal malignancies(27), Urogenital malignancies(22),Leukemia(16), Carcinoma Lung(14),Lymphoma(11),Head and neck cancers(8),Bone tumours(4), Carcinoma breast(12)Neurological malignancies(2), malignant melanoma(1),Choriocarcinoma(1),and Carcinoid(1). The other group of 21 patients with othermonocloanl gammopathies were also studied which included patients with infection(7)and patients with immunological disease(14). The study revealed an increase in the total serum LD activity with an increase in LD, fraction and decrease in LD2 fraction. The study indicates that serum total LD and isoenzymes pattern may be of diagnostic importance in neoplastic disorders encountered in clinical practice.

Lymphocytic lymphoma of eye-lids.

A 46 years old man presenting with swelling of both upper lids, was found to have well differentiated lymphocytic lymphoma. He is on external beam irradiation (teletherapy)and has shown satisfactory response.

Malignant fibrous histocytoma of the lung.

Primary malignant histocytoma ofthe lung is a rare disease. We report the clinical features and management of a patient with recurrent malignant fibrous histocytoma involving the left lung. The patient had a recurrent tumour following treatment leading to troublesome dysphagia due to extrinsic compression of the lower esophagus which was relieved by placement of an endoprosthesis.
